How the billing works
DiaSante is engagement-based: the provider bills Medicare or the patient’s insurance for the covered services, and DiaSante supplies the devices, staffing, and documentation that make those claims defensible.
The relevant code families are RPM (99453 / 99454 / 99457 / 99458), CCM (99490 / 99491 / 99439), PCM (99424–99427), and, for FQHCs and RHCs, the G0511 care-management code. Because the care team and the data record are built around those codes from day one, the monthly documentation is audit-ready rather than reconstructed after the fact.
